Simply download the petition, fill in your information and fill out the bottom affidavit. Check out our step by step guide to ensure your signature counts!
STEP 1
Print the petition DOUBLE SIDED
The petitions must be printed as a two-sided (front-back) document in order to be valid and get counted. You will find both of these petitions enclosed in the envelope you received in the mail.
Write your FULL NAME as it appears on your voter registration
Sign your full legal name as it appears on your voter registration. All components of your name as registered (last name, first name and the initial of any middle or other name) must be included in both the printed name and signature portions of the petition entry to be valid. The printed name is not required to match the signature, but the names must be consistent.
STEP 3
Write your PERMANENT RESIDENCE Address
A business address or PO Box is insufficient.
Residents can be registered voters even without a residence if they live and receive mail in Baltimore City.
When signing a petition, you should use your voter registration address.You are required to write your 1) Street 2) House and/or apartment number 3) City & 4) Zip Code
STEP 4
Write your Date of Birth
Each registered voter is asked to include his/her date of birth on the petition.
STEP 5
Sign the Petition with your signature and date it!
Sign as your name appears in voting records: SIGN YOUR FIRST NAME SIGN YOUR LAST NAME INCLUDE YOUR MIDDLE INITIAL IF YOU HAVE ONE
DON'T FORGET TO DATE YOUR SIGNATURE
STEP 6
Don't forget to sign the Affidavit
At the bottom of the petition page on the left-hand side, where it says “Individual Circulator’s printed or typed name,” add: – your printed name – your address (street number and name, city, state, and zip code), – and your phone number (including the area code).
At the bottom of the petition page on the right-hand side, where it says “Circulator’s Affidavit,” sign and date on the line.
STEP 7
Fold, Stamp, and Mail!
